Physician notes regarding Physical Education (PE) or athletics should be presented to the Nurses' office. If appropriate, PE teachers, coaches and athletic trainers will be informed in compliance with FERPA. Students will be asked if a trainer or coach needs a copy. All physician notes will be maintained in the student’s health file in the Nurses' office. 

Any physician notes regarding student absences will be forwarded to the Attendance Office as appropriate.

When a student has sustained an injury or illness that requires the use of the elevator key/keycard, it can be obtained in the Nurses' Office. A doctor’s note is required in order for an elevator key/keycard to be issued. Physician notes should include an estimated timeframe the student will be out of school/PE. A $25 replacement charge will be assessed for a lost key/keycard.
