If you need a copy of your current student’s records, contact the Nurses' Office at the campus your student attends. Any student over the age of 18 must request their own records, per FERPA.

 

For students who have graduated, contact the Nurses' Office at Lake Park West Campus. 
 

When requesting health records, send an email to rriggio@lphs.org or kschenone@lphs.org with the following information:

1. Student's name and date of birth
2. Student ID#
3. The year that your student will graduate/has graduated
4. Contact person's name and phone number

Records can only be provided to the requestor. Please indicate a preference how to receive this information: in-person pick up, USPS to the requestor's home address, email address, or via fax.
