  • The CERP Protocol is specific facility-based actions taken in a cardiac emergency that can be shared with team members and visually posted throughout a building. This document is supported by a national task force of experts in preparing for sudden cardiac arrest, the American Heart Association, Project ADAM, and Parent Heart Watch.

  • *Medications will only be administered with appropriate forms on file in the Nurses' Office.* 

    Two medication forms are available for downloading and are also available in the Nurses' Office:
    Daily and As Needed Medication Authorization Form
    Emergency/Self-Administration Medication Authorization Form
     
    The forms need to be renewed yearly. All medications including prescribed and over-the-counter require the medication authorization form to be completed by both physician and parent.  The medication must be brought to the Nurses' office in its original container. Controlled substances (i.e. ADHD medicine) must be brought to the Nurse by a parent/guardian. At that time, the medication will be counted, logged, and authorized by the Nurse and parent/guardian. If a prescribed medication is required at both home and at school, request that the pharmacy provide two labeled bottles.

    All medications need to be picked up at the end of the school year or will be disposed of appropriately. Students may pick up any remaining medication at the end of the school year; however, a parent/guardian must pick up any remaining controlled substances. If you have any questions, contact the school Nurses' Office.

     

  • With the exception of an emergency, all students who come to the office for an illness must have a pass from the class they are missing. Students are not allowed to come in during the passing period. Each student is asked to sign in and then will see the nurse or health aide. The student will be assessed to determine appropriate action to be taken. In order for a student to be released to go home, the Nurses' Office staff must speak with a parent/guardian or an emergency contact who has been designated by the parent.

    Student Release Procedure: East Campus
    A parent/guardian/emergency contact must sign the student out at the security desk. 
     

    Student Release Procedure: West Campus
    A parent/guardian/emergency contact may sign the student out at the security desk. 

    A parent/guardian/emergency contact must give verbal consent for the student to drive/walk home at the discretion of the Nurse. 

  • Physician notes regarding Physical Education (PE) or athletics should be presented to the Nurses' office. If appropriate, PE teachers, coaches and athletic trainers will be informed in compliance with FERPA. Students will be asked if a trainer or coach needs a copy. All physician notes will be maintained in the student’s health file in the Nurses' office. 

    Any physician notes regarding student absences will be forwarded to the Attendance Office as appropriate.


    When a student has sustained an injury or illness that requires the use of the elevator key/keycard, it can be obtained in the Nurses' Office. A doctor’s note is required in order for an elevator key/keycard to be issued. Physician notes should include an estimated timeframe the student will be out of school/PE. A $25.00 replacement charge will be assessed for a lost key/keycard.

  • If you need a copy of your current student’s records, contact the Nurses' Office at the campus your student attends. Any student over the age of 18 must request their own records, per FERPA.
     
    For students who have graduated, contact the Nurses' Office at Lake Park West Campus. 
     
    When requesting health records, send an email to rriggio@lphs.org or kschenone@lphs.org with the following information:

    • Student's name and date of birth
    • Student ID#
    • The year that your student will graduate/has graduated
    • Contact person's name and phone number

    Records can only be provided to the requestor. Please indicate a preference how to receive this information: in-person pick up, USPS to the requestor's home address, email address, or via fax.

  • Illinois State law requires that the following high school students be screened for vision and hearing on an annual basis:

    • special education students
    • transfer students
    • foster students 
    • teacher/parent/guardian referrals

    The screening process, administered by a certified vision and hearing technician, will begin in September and continue throughout the school year.

    Vision screening is not a substitute for a complete eye and vision evaluation by an eye doctor.  Your child is not required to undergo this vision screening if an optometrist or ophthalmologist has completed and signed a report form indicating that an examination has been administered within the previous 12 months.
