Work Permits

Work Permit Process

Complete a work permit application. Bring the completed application with the required documents to Lake Park.
A parent/guardian must be present when the student is applying for the permit.  

Documents needed for Lake Park students:

  • Completed application
  • Letter of intent from employer on letterhead
  • Original social security card 

Documents needed for non-Lake Park students:

  • Completed application
  • Letter of intent from employer on letterhead
  • Original social security card
  • Original birth certificate
  • Physical dated within one year
  • Driver's license or state ID of parent/guardian
  • Proof of Coogan or UTMA account
    • Applies to minors applying for a performing, acting or modeling work permit.
  • Principal’s statement (waived during summer months when school is not in session)

The work permit will be submitted online to the Department of Labor. Parents/Guardians/student will be provided two copies, one for the employer and one to keep for their records.
