Transcript Requests
Student transcript request must be in writing including the student’s signature. Please download the request form by clicking here. Request must be mailed to the Registrar, Lake Park High School West Campus, 500 W. Bryn Mawr Ave. , Roselle , IL 60172 . Faxes or email will not be accepted. Please include $1.00 per transcript and allow a turn-around time of 2 – 3 weeks.
